  • High School Card (Form 138)
  • Certificate of transfer (for Transferees)
  • Certificate of Transfer (for Transferees)
  • Birth Certificate (PSA Authenticated)
  • Marriage Certificate (PSA Auth.) (if applicable)
  • Form 177 / TOR Copy for UPHS Pueblo de Panay Campus


  • Additional Requirements for Senior High: ESC / QVR Certificate

Enrollment Procedure
  • 1. Information. Information Officer. Secure List Requirements, Locator number.
  • 2. Testing Center. SPS Personnel. Take Entrance Exam and get results.
  • 3. Admission & Advising. Registrar. Submission and Validation of Requirements.
  • a. Advising and Pre-Assessment of Subjects, and Selection of Payment Scheme.
  • 4. Payment of Fees. Cashier. Pay assessed tuition other fees and Enrollment Kit.asdfasdf
  • 5. Registration Form. Registrar. Present receipt and get Official Registration Form and Enrollment Kit.
  • 6. ID. IT Personnel. Present registration form for ID Picture taking.
