- High School Card (Form 138)
- Certificate of transfer (for Transferees)
- Certificate of Transfer (for Transferees)
- Birth Certificate (PSA Authenticated)
- Marriage Certificate (PSA Auth.) (if applicable)
- Form 177 / TOR Copy for UPHS Pueblo de Panay Campus
- Additional Requirements for Senior High: ESC / QVR Certificate
- a. Advising and Pre-Assessment of Subjects, and Selection of Payment Scheme.
